5. Marriage Certificate (for married applicant)
6. Two valid ID's (photocopy)
7. 2 x 2 picture with white background (1 copy)
8. Accomplish Application Form (signed by Two Voting Members)
9. Entrance Fee: Php 100,000.00
                            Php 50,000.00 (50% discount if child of member for more than 10 years)

Requirements for JUNIOR MEMBERSHIP
1. Applicants must be single within 21 to 30 years of age
2. Accomplish Undertaking
3. Proof of Billing Address (Latest)
4. Sketch of House and Office Address
5. Photocopy (clear) of Birth Certificate of the Applicant
6. Two valid ID's (photocopy)
7. 2 x 2 picture with white background (1 copy)
8. Accomplish Application Form (signed by Parent and Another Members)

Requirements for COMPANY MEMBERSHIP
1. Purchase of Casino Español De Cebu Share
2. Proof of Billing Address (Latest)
3. Sketch of House and Office Address
4. Photocopy (clear) of Birth Certificate of the Applicant
5. Marriage Certificate (for married applicant)
6. Two valid ID's (photocopy)
7. 2 x 2 picture with white background (1 copy)
8. Secretary's Certificate & Board Resolution for representative & must be duly notarized.
9. Certificate of Registration, including Articles of Incorporation, By-Laws
10. For companies outside the Philippines, comparable documents are to be obtained, duly authenticated by the Philippine Consulate where said companies are located
11. Accomplish Application Form (signed by Two Voting Members)
12. Entrance Fee: Php 150,000.00

Requirements for ASSIGNEE MEMBERSHIP
1. Deed of Assignment
2. Turn over the Orginal Proprietary Certificate of the shareholder
3. Proof of Billing Address (Latest)
4. Sketch of House and Office Address
5. Photocopy (clear) of Birth Certificate of the Applicant
6. Marriage Certificate (for married applicant)
7. Two valid ID's (photocopy)
8. 2 x 2 picture with white background (1 copy)
9. Accomplish Application Form (signed by Two Voting Members)
10. Entrance Fee: Php 100,000.00 + Php 30,000.00 refundable deposit = Php 130,000.00
                              Php 50,000.00 (50% discount if child of member for more than 10 years)
